ACH Scholarship Award Notification

UW-Madison is now able to accept ACH transactions from organizations submitting student scholarship awards. Please complete the following required information, submit the ACH Scholarship Award Notification Spreadsheet using our template, and agree to the UW-Madison Scholarship Policy. Once this is complete, we will send you an email with our ACH banking instructions.

Upload the Scholarship Notification Spreadsheet listing the scholarship recipients using the template provided (top of page). This spreadsheet must include the student's full name, their 10-digit Campus ID number, the amount of the scholarship, and the term (Fall only, Spring only, or split Fall/Spring) that the funds should be applied to.

By submitting this form, I understand that UW-Madison will post the payment according to the instructions provided. UW-Madison will post the scholarship for the amount received. I understand that UW-Madison is not able to monitor student academic information (such as, but not limited to, full-time enrollment or GPA). If the student is not enrolled, the funds will be returned to the organization. However, if the student withdraws during the term and receives a refund, it is the student’s responsibility to return any funds to the organization. UW-Madison will not monitor if the student's account balance is zero and the scholarship may be refunded to the student. All scholarships are included in the calculation of unmet need.
